****$5,000 SIGN ON BONUS****

Under the direction of the Director of Patient Care Services, the Hospice Clinical Manager monitors/assures, supervises and directs field staff & clinical office staff activities; is responsible for field staff competencies, and their safe and efficient comprehensive end of life care delivery to patients. The manager supervises a variety of clinical roles which most often include nurses, aides, social workers, spiritual counselors, musicians, and dieticians.

CORE PERFORMANCE REQUIREMENTS may include but are not limited to:

• Monitors and maintains productivity standards for all field staff positions.
• Demonstrates effective problem solving, good judgment, and confidential handling of sensitive information.
• Demonstrates accurate and timely documentation in accordance with established regulations.
• Facilitates the establishment and maintenance of successful Clinician/Patient relationships as evidenced by achieving an agency average of 90% or above with HHCAHPS satisfaction rates working with the QAPI staff on trends and needed education.
• Assists management team in facilitating successful licensing, certification and accreditation surveys and assists in developing/implementing any corrective action.
• Oversees the scheduling and/or team assistant, assisting with scheduling to ensure appropriate clinical staffing as evidenced by clinical staff achieving/exceeding productivity standards.

QUALIFICATIONS AND REQUIREMENTS

• Graduation from an approved school of professional nursing.
• Current licensure by the state of California as a Registered Nurse with at least 2 years of experience in home health/hospice or community health within the last 5 years.
• Demonstrated knowledge of and ability to apply advanced principles of nursing care.
• Demonstrated leadership ability, teaching skills, and the capability to work effective with all levels of personnel.
• Must be C.P.R. Certified.
