Holy Family Home (Little Sisters of the Poor) is a Long-Term Care facility that is dedicated to caring for the elders. Holy Family Home provides our residents with high quality and caring services. Holy Family Home is responsible for the care, comfort and safety of our residents in all activities of daily living. Holy Family Home is dedicated to treating our patients and each other as family as well as ensuring we provide a comfortable and friendly place to work.

Holy Family Home is looking for amazing, full and part time Registered Nurses (RN), Licensed Practical Nurses (LPN) and RN Supervisor to join our family team to work full time or part time - 2 - 5 days per week including every other weekend.

Position requires you to be Covid-19 vaccinated, and flu vaccinated.

JOB DESCRIPTION for RN:

Responsible for the provision and the supervision of medical and personal care services in assigned area, to attain or maintain the highest practicable mental and psychosocial well-being in accordance with the comprehensive assessment and plan of care. These services are consistent with the Home's standards and philosophy and are in compliance with all regulations, codes, policies and procedures that govern the facility.

  • Must be a graduate of an accredited School of Nursing, licensed to practice in Pa as a Registered Nurse.
  • Must have one-year experience as a nurse, preferably in the area of geriatrics, rehabilitation or psychiatric nursing.
  • Must possess good written and verbal communication skills. Must be able to read, write and understand English.
  • Must be able to function independently performing related duties/tasks with a minimum of supervision or direction.
  • Must have a basic knowledge of State and Federal regulations and guidelines governing long term care.
  • Must have good assessment skills and a working knowledge of the nursing process.
  • Preferable previous supervisory or management experience.
  • Must possess supervisory and leadership skills to interact effectively with staff, Residents and families.
  • Must possess the ability to perform *essential job functions with or without reasonable accommodations.

JOB REQUIREMENTS for RN:

  • Assess and evaluate assigned Residents daily and report to relief nurse.
  • Timely reporting of changes in Residents status or condition to Director of Nursing or her designee.
  • Notify Resident physician and responsible party of untoward change of Resident status, with prior reporting (if possible) to Director of Nursing or Mother Superior.
  • Assist Resident with issues related to psychosocial needs i.e. aging and health changes/illness, loss and grief.
  • Administer and document prescribed medications/treatments accurately and timely and in compliance with policies/procedures.
  • Respond calmly, quickly to Resident and employee emergencies reporting to Director of Nursing or her designee.
  • Transcribe accurately and timely all physician orders.
  • Participate in the comprehensive Resident assessment and update the plan of care in a timely manner to assure its accuracy and appropriateness.
  • Prepare for and assist with Physician visits to the Home.
  • Assist with orientation of Nursing Assistants and/or select nursing personnel as indicated.
  • Supervision and follow up of Nursing Assistant care and assignments.
  • Participate in the evaluation of Nursing Assistants as indicated.
  • Report to the Director of Nursing or her designee any unresolved or pertinent issues with Nursing Assistants.

JOB DESCRIPTION for RN Supervisor

Assists the Director of Nursing in assuring that nursing service objectives, standards of nursing practice, and quality of care comply with the Home. Assures that the provision of care to each Resident meets their physical, social, emotional, and spiritual needs consistent with the comprehensive assessment and plan of care. These services are consistent with the Home’s standards and philosophy and comply with all regulations, codes, policies and procedures that govern the facility.

  • Must be a graduate of an accredited School of Nursing, licensed to practice in Pa. as a Registered Nurse (RN).
  • Must have a minimum of three years’ experience as a Registered Nurse.
  • Must be able to assume responsibility for the duties of the Director of Nursing in her absence.
  • Must possess good written and verbal communication skills. Must be able to read, write and understand English.
  • Must be able to function independently performing related duties/tasks with a minimum of supervision or direction.
  • Must have a thorough knowledge of State and Federal regulations and guidelines governing long-term care facilities.
  • Must possess maturity and leadership ability to interact effectively with staff, Residents, and families.
  • Must possess management and supervisor skills.
  • Possess the ability to perform the *essential job functions with or without reasonable accommodations.

JOB REQUIREMENTS for RN Supervisor:

  • Keep Director of Nursing informed of all nursing concerns and assist in creative problem solving.
  • Review and evaluate all Residents' clinical records (in collaboration with the physicians) to assure the medical plan of care comply with Federal and State regulations.
  • Review nursing documentation (nursing summaries, follow-up charting, acute changes in condition, daily unit census, etc.) to assure compliance with regulations and that charting presents an accurate, comprehensive and objective clinical picture.
  • Notify of physician and/or responsible party regarding Resident status as warranted.
  • Coordinate physician visits and supervise adherence to regular schedule. Daily coordinate physician calls after consultation with unit nurses.
  • Frequently visit Residents to assure optimal care, assessing and documenting changes in condition and informing of physician and family.
  • Review all consultations; assure that follow-up is complete and updated.
  • Oversee the completion of the comprehensive Resident assessment and assure carry over/update to the plan of care in a timely manner to assure its accuracy and appropriateness.
  • Assist with orientation of all new nurses to policies and procedures of the Home.
  • Assure completion of new admission and readmission order. Audit all closed clinical records, assuring complete and accurate documentation.
  • Communicate with pharmacy frequently to assure staff compliance with Pharmacy Policy and Procedure and State and Federal regulations.
  • Maintain confidentiality of all Resident, departmental and organizational information. Demonstrate behavior that indicates recognition of Resident rights.
  • Work effectively and cooperatively in role as leader/supervisor within department and with all departments or other disciplines.
  • Follow all safety, security, infection/exposure control and hazardous waste policies and procedures (including Category I universal precautions).

  • Able to perform simple manipulative skills such as, writing, filing, etc.
  • Able to lift and move objects weighing 50lbs
  • Able to walk and stand constantly, frequently climb stairs, squat, kneel, twist, sit, bend and reach overhead.
  • Maintain CONFIDENTIALITY of all Home information.
  • Work effectively and cooperatively as a team member within Department and with all departments or other disciplines.
  • Observe dress and conduct codes, including wearing identification badge.
  • Meet attendance standards as established and provide proper notice of absence.
  • Complete duties in prescribed time.
  • Attend in-services and department meetings.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment in the reception area.
  • Perform other duties as assigned by supervisor

WORK SETTING/ENVIRONMENT:

Work is performed primarily indoors but at times may attend appointments outside the facility or may participate in outdoor activities of the Home. Work environment is well lighted, clean and suitable to tasks performed by the employee within temperature control standards that govern a skilled care facility.
